LAFAYETTE, Ore. - A man died after a head-on crash on Highway 99W Sunday evening.
    
The collision occurred just after 5 p.m. Sunday in Lafayette, which is north of McMinnville.
    
Police say a Pontiac driven by 21-year-old Braulio Juarez collided head-on with an SUV.
    
The 19-year-old passenger in the car, identified as Jose Juarez Cuellar, died at the scene.
    
Several people in the SUV received non-life threatening injuries, including three children.
    
Police are still investigating what caused the collision.
